If you meet certain criteria, the Delaware General Health District is offering COVID-19 testing at a pop-up drive-thru clinic which will be located within the city of Delaware.

This service is for those with COVID-19 symptoms and are without health insurance and/or a primary care provider.

Testing will be held by appointment only on May 28 from 1 PM to 3 PM.

These instructions will apply:

  • At least two days prior to the clinic, go to this Form.
  • Enter “covid19” in the Consent Form Code box
  • Complete the electronic consent
  • Sign with a digital signature
  • Click “Submit”

You will receive notice the day before the clinic of your approval/disapproval and appointment time. This is an appointment-only clinic, no walkups will be accepted.
