The Delaware Public Health District (DPHD) is proud to unveil its new Care Coordination Program, an innovative service aimed at improving health outcomes for individuals and families in Delaware County. The program’s debut coincides with the arrival of Krista Allen, DPHD’s first-ever Community Health Worker (CHW) (pictured above).
Supporting Health Through Care Coordination
The Care Coordination Program provides personalized assistance to help residents navigate the complex web of health, social, and economic resources available in the community. Community Health Workers, such as Krista Allen, are the cornerstone of this effort. Trained and employed by Care Coordination agencies, CHWs offer one-on-one support tailored to each client’s unique needs.
Krista Allen expressed her enthusiasm for the role, saying, “I’m excited to have an opportunity to help our clients navigate resources throughout Delaware County. I look forward to building a relationship with our partnerships and clients.”
Allen’s work will focus on bridging the gap between individuals and the resources they need, ensuring no one is left to face challenges alone.
Strengthening Community Health Partnerships
Deputy Health Commissioner Jen Keagy highlighted the broader impact the program will have, both for clients and for the district’s partnerships.
“The Care Coordination Program will allow us to strengthen relationships with our partners, avoid duplication of services, and address the social determinants of health,” Keagy explained. “Our CHW will walk with individuals and families along their journey to navigating their healthcare needs, with the goal of improving health outcomes and their quality of life.”
The program emphasizes collaboration with local Pathway Hub agencies, ensuring that services are not only effective but also streamlined, avoiding overlap or gaps in care.
What Community Health Workers Offer
Community Health Workers are much more than navigators. They serve as advocates, guides, and trusted allies in addressing the social determinants of health—factors like housing, education, access to food, and economic stability that profoundly influence well-being. Their work aligns clients with the services and programs necessary to achieve stability and health security.
