The Delaware County Community Health Improvement Plan (CHIP) has been completed and is posted for public comment on the Delaware General Health District’s website.

The Health District is asking community residents to visit this site to review the action plan and post any comments or suggestions by November 19. The CHIP is available for public comment on the Plans and Data page of DelawareHealth.org. Residents can review the document and post any comments, concerns or questions.

The Partnership for a Healthy Delaware County (The Partnership) wrapped up the 2018 CHIP planning process commissioned by the Health District this fall.[widgets_on_pages id=1]

The Partnership determined these health issues as the top priorities facing Delaware County:

  • Chronic Disease
  • Mental Health and Addiction
  • Cross-Cutting Strategies

Partnership members and other community members met over the summer to develop goals and strategies to address the identified priorities. The Partnership then compiled those strategies into the CHIP.

The Partnership for a Healthy Delaware County is a multi-sector group consisting of approximately 150 members, including community leaders, residents, organizations and agencies. A total of 72 partners participated in drafting the health improvement plan.
